GIC Health Insurance Open Enrollment:

Annual Open Enrollment for GIC Health Insurance runs from APRIL 3rd, 2019 to MAY 1st, 2019. Enrollments/Changes made during this period are effective on July 1st 2019.

Should you choose to enroll or make a change, please complete the applicable form below, attach all required documentation, and submit it to the Human Resources Department on or before May 1st, 2019.

Altus Dental Insurance Open Enrollment:

Open Enrollment for Altus Dental Insurance runs April 1st, 2019 to May 1st, 2019. Enrollments/Changes made during this period go into effect on July 1st, 2019. Altus Dental Insurance is available to benefited City employees. Please refer to the benefit highlight sheet and enrollment form below.
